The Oklahoma Medicaid program is SoonerCare; SoonerCare Choice is the managed care component of SoonerCare. How Can I Apply For The SoonerCare?

  • To apply for Soonercare, you need to complete the online application form.
  • Soonercare no longer accepts paper applications, but you can download them from http://healthcare.gov/ if you need to. You can also call 1-800-318-2596 to request your paper application.
  • In the case of a paper request, you must send it by post to the address indicated on the form.
  • However, not everyone can access the Internet to complete the application or download and fill out the application form online. In cases where people are receiving Social Security Income (SSI) or TANF (Temporary Aid for Needy Families) cash aid, they should contact the DA office directly.

When applying online, you must slowly fill in all the program details. Some of them are the following information

  • Children under the age of 19 accompanied by an adult may participate in Soonercare programs.
